The slope intercept form a linear equation has the form
y = mx + b

In getting the slope-intercept form of a linear equation, the following should be present: Slope (m) and the x-intercept (b). 

If you are given only two coordinates or two points, the the steps are:
1. Get the slope (m) using the formula m = (Y1-Y2) / (X1-X2).
2. Get the x-intercept (b) by substituting any of the two points to the slope-intercept equation.
